Living Will

An advance directive or "living will," is a document that tells your care providers who should speak for you and what medical decisions they should make if you become unable to speak for yourself. You can attach a personal letter to this form if you would like to. It’s important that you give copies of this form to your care team, family members, & funeral home if you are using one.

There is a $10 fee to register your Advance Health Care Directive with the State. After the documents are filed, a Registry Card containing a unique file number and password will be issued to the applicant. The applicant can use the file number and password to access documents online.

Do Not Resuscitate

This form instructs medical personnel not to use CPR, electric shock to the heart, artificial breathing devices, or other procedures to revive you if you stop breathing or your heart stops beating. This is non mandatory and dependent on personal preference.

Medical POA

This form appoints a representative you can trust to make medical decisions for you should you become incapacitated.
